At Bloom Care Management our case managers work closely with individuals and their families to:

How do I get started?

Bloom Care Management is contracted with Community Mental Health Authority of Clinton, Eaton, and Ingham Counties (CMHA-CEI). If you are in need of Case Management services, please reach out to CMHA-CEI’s Access department to schedule an intake assessment.
